Membership Application

People and organizations requesting membership must fill out this form in its entirety and then submit it. Please recognize that the PMTA Code of Conduct and Non-Disclosure Agreement must be read and reviewed prior to completion of this application.

Please outline how your organization ensures that students receive a quality learning experience. You may emphasize how content is delivered, how you select and monitor instructors, or how you follow up with students. You must include at least a 150-word description in order for your application to be considered.
